A customer service representative will assess your situation and send you the relevant claims package.

Step 3

You complete the forms & return them

Once you have completed the forms, return them to us by secure email, fax or by mail in the self-addressed, stamped envelope provided.

Note: incorrect information or missing documents can delay a claim decision so please carefully review the materials to ensure that you have all the required documentation before sending them to us.
